2224. A Do-It-Yourself Revolution in Diabetes Care

John Costik got the call at the office in 2012. It was his wife, Laura, with terrible news: Their 4-year-old son, Evan, was headed into the emergency room.

His blood sugar reading was sky high, about 535 mg/dl, and doctors had discovered he had Type 1 diabetes. The first three days in the hospital were a blur during which the Costiks, engineers in Rochester, received a crash course in managing the basics of diabetes care.

For starters, they were told to log their son’s numbers on paper forms. It was their first hint that diabetes management did not occupy a place on technology’s bleeding edge. The methods for guesstimating carbohydrate intake also seemed imprecise, Mr. Costik found, and the process generated a lot of wasted data.

“The last thing you want to do is find some form and fill it out,” he said. “You’re really just emotionally trying to cope with it, and that data in that book isn’t necessarily useful to the people with diabetes.”

Several months later, Mr. Costik fitted his son with a Dexcom G4 continuous glucose monitor. A hair-thin sensor under Evan’s skin recorded an exact blood sugar reading at five-minute intervals, 24 hours a day.

But all that data left with Evan every morning when he headed off to day care. Mr. Costik wanted something better: continuous access to his son’s glucose readings.

So he examined the device’s software code and wrote a simple program that transmitted the monitoring data to an online spreadsheet he could view on a Web browser, Android mobile phone or, eventually, his Pebble smartwatch.

“I wanted our lives to be simple,” Mr. Costik said, “and I wanted Evan to live a long time, and diabetes to be a nuisance, not a huge struggle.”

Mr. Costik shared a photograph of his simple hack on Twitter — and discovered a legion of parents who were eager to tailor off-the-shelf devices into homemade solutions. 

Together, they have set in motion a remarkable, egalitarian push for improved technology to manage diabetes care, rarely seen in the top-down world of medical devices.

In 2014, the last year for which data is available, the Centers for Disease Control and Prevention estimated that 29 million adults were living with diabetes. Of these, 5 to 10 percent had Type 1, which develops when the body’s immune system destroys pancreatic beta cells.

Now, as consumer gadgets weave themselves ever more tightly into everyday life, patients and their families are finding homespun solutions to problems medical-device manufacturers originally did not address. Industry executives say the pace of user-driven innovation was one reason the Food and Drug Administration recently reclassified remote glucose-monitoring devices, hastening approval for new models by big companies like Dexcom and Medtronics.

James Wedding, a civil engineer who lives outside Dallas, saw Mr. Costik’s Twitter post and used his code to set up a remote monitor system for his daughter, Carson, who is now 12.

“Once I got all the pieces together, I remember crying — not quite in sadness, just in utter amazement — the first time I could see her numbers displayed on my computer screen and she was on the other side of the house,” Mr. Wedding said.

“It is such a change in your relationship when the first question out of your mouth when you talk to your son, your daughter, your spouse, your brother, whatever, is no longer, ‘Hey, what’s your number?’ It’s ‘How was math class? How was work? What are you up to today?’”

Lane Desborough, an engineer in California, got in touch with Mr. Costik after seeing his tweet, ultimately creating an open-source system based in part on Mr. Costik’s code. It allows anyone to hack existing glucose monitors so they transmit readings to the cloud, where they can be read by patients and caregivers.

Mr. Desborough called the project Nightscout. The Nightscout group on Facebook, known as CGM in the Cloud, provides free tech support for users trying to improve on monitoring devices.

About two dozen users have even started a project called Open APS, in which they are pairing insulin pumps with glucose monitors in an effort to create an open-source artificial pancreas system. These wearable devices, which automate insulin delivery, are being tested in academic settings, but these early adopters are not waiting for the results of those continuing clinical trials.

Mr. Costik now works at the Center for Clinical Innovation at the University of Rochester, where he works to improve management options for all patients; Mr. Desborough is now the chief engineer at Bigfoot Biomedical, a start-up in Palo Alto, Calif., that plans to create an artificial pancreas.

More recently, the home tinkering projects have buoyed a patient-led initiative to make generic insulin. Anthony Di Franco, a founder of the biotech hacker space Counter Culture Labs in Oakland, Calif., has had diabetes for 10 years. Mr. Di Franco saw what parents with diabetic children were doing with glucose monitoring devices and wondered why, even with insurance coverage, a three-month supply of insulin often totaled hundreds of dollars.

“I was frustrated with the situation,” he said.

With available laboratory tools, and a wealth of available academic literature, he set out to learn whether insulin could be home-brewed on a small scale. After some research, Mr. Di Franco realized, “We can do it, and we can do it now. All of the tools already exist.”

Last year, the Open Insulin Project raised $16,656 in one of the more ambitious efforts to radically transform diabetes care. So far, the small team of researchers has inserted the genes that make proinsulin (the form of insulin produced by the human body) into E. coli bacteria and began culturing the organism on a larger scale.

The intent is not to make insulin at home, or on an industrial scale. Any drug that is injected comes with substantial risks and would face considerable regulatory scrutiny. Rather, the hackers hope to be able to demonstrate the technological feasibility. Within a year or two, Mr. Di Franco said he envisions handing off the protocols and any intellectual property to a generics manufacturer.

“One thing that would make me happy,” he said, “is that if more people who needed insulin got ahold of it by whatever means necessary.”

Dr. Jeremy A. Greene, a physician and historian at Johns Hopkins University, who recently wrote in The New England Journal of Medicine about the lack of generic insulin, said patients with diabetes had a long history of tinkering with existing technology, even in ways that were not officially sanctioned.

Dr. Greene argues that while manufacturers in insulin are making innovations — the newest forms of insulin are substantial improvements over earlier products — they stop producing the older forms once they lose patent protection. Patients and their insurers pay a high price for patented insulin or go without.

Biohackers are attempting to resurrect an older product to address the lack of generic insulin, Dr. Greene said.

“I don’t think that we should be surprised that a population of technologically savvy patients, whose lives are dependent on access to a supply of a biological agent, should be interested in taking means of production into their own hands, especially at time when insulin prices have risen at unpredictably alarming rates.”

2221. Pope Francis Appeals for Global End to Capital Punishment

Pope Francis has called for the worldwide abolition of the death penalty, saying the commandment “You shall not kill” absolute and valid for the guilty as well as the innocent.

Using some of his strongest ever words against capital punishment, he also called on Catholic politicians worldwide to make “a courageous and exemplary gesture” by seeking a moratorium on executions during the church’s current holy year, which ends in November.

“I appeal to the consciences of those who govern to reach an international consensus to abolish the death penalty,” he told tens of thousands of people in St Peter’s Square in the Vatican on Sunday.

“The commandment ‘You shall not kill’ has absolute value and applies to both the innocent and the guilty,” he told the crowd.

The 1.2 billion-strong Catholic church allowed the death penalty in extreme cases for centuries, but the position began to change under the late Pope John Paul, who died in 2005.

The pope added there was now “a growing opposition to the death penalty even for the legitimate defence of society” because modern means existed to “efficiently repress crime without definitively denying the person who committed it the possibility of rehabilitating themselves”.

Francis made the comments to support an international conference against the death penalty starting on Monday in Rome and organised by the Sant’Egidio community, a worldwide Catholic peace and justice group.

Francis, who has visited a number of prisons since his election as pope nearly three years ago – the most recent in Mexico last week – also called for conditions to be improved.

“All Christians and men of good will are called on to work not only for the abolition of the death penalty, but also to improve prison conditions so that they respect the human dignity of people who have been deprived of their freedom,” he said.

In the past, the pope also denounced life imprisonment, calling it “a hidden death penalty”. He said more should be done torehabilitate even the most hardened of criminals.

2220. Seas Are Rising at Fastest Rate in Last 28 Centuries

The worsening of tidal flooding in American coastal communities is largely a consequence of greenhouse gases from human activity, and the problem will grow far worse in coming decades, scientists reported Monday.

Those emissions, primarily from the burning of fossil fuels, are causing the ocean to rise at the fastest rate since at least the founding of ancient Rome, the scientists said. They added that in the absence of human emissions, the ocean surface would be rising less rapidly and might even be falling.

The increasingly routine tidal flooding is making life miserable in places like Miami Beach; Charleston, S.C.; and Norfolk, Va., even on sunny days.

Though these types of floods often produce only a foot or two of standing saltwater, they are straining life in many towns by killing lawns and trees, blocking neighborhood streets and clogging storm drains, polluting supplies of freshwater and sometimes stranding entire island communities for hours by overtopping the roads that tie them to the mainland.

Such events are just an early harbinger of the coming damage, the new research suggests.

“I think we need a new way to think about most coastal flooding,” said Benjamin H. Strauss, the primary author of one of two related studies released on Monday. “It’s not the tide. It’s not the wind. It’s us. That’s true for most of the coastal floods we now experience.”

In the second study, scientists reconstructed the level of the sea over time and confirmed that it is most likely rising faster than at any point in 28 centuries, with the rate of increase growing sharply over the past century — largely, they found, because of the warming that scientists have said is almost certainly caused by human emissions.

They also confirmed previous forecasts that if emissions were to continue at a high rate over the next few decades, the ocean could rise as much as three or four feet by 2100.

Experts say the situation would then grow far worse in the 22nd century and beyond, likely requiring the abandonment of many coastal cities.

The findings are yet another indication that the stable climate in which human civilization has flourished for thousands of years, with a largely predictable ocean permitting the growth of great coastal cities, is coming to an end.

“I think we can definitely be confident that sea-level rise is going to continue to accelerate if there’s further warming, which inevitably there will be,” said Stefan Rahmstorf, a professor of ocean physics at the Potsdam Institute for Climate Impact Research, in Germany, and co-author of one of the papers, published online Monday by an American journal, Proceedings of the National Academy of Sciences.

In a report issued to accompany that scientific paper, a climate research and communications organization in Princeton, N.J., Climate Central, used the new findings to calculate that roughly three-quarters of the tidal flood days now occurring in towns along the East Coast would not be happening in the absence of the rise in the sea level caused by human emissions.

The lead author of that report, Dr. Strauss, said the same was likely true on a global scale, in any coastal community that has had an increase of saltwater flooding in recent decades.

The rise in the sea level contributes only in a limited degree to the huge, disastrous storm surges accompanying hurricanes like Katrina and Sandy. Proportionally, it has a bigger effect on the nuisance floods that can accompany what are known as king tides.

The change in frequency of those tides is striking. For instance, in the decade from 1955 to 1964 at Annapolis, Md., an instrument called a tide gauge measured 32 days of flooding; in the decade from 2005 to 2014, that jumped to 394 days.

Flood days in Charleston jumped from 34 in the earlier decade to 219 in the more recent, and in Key West, Fla., the figure jumped from no flood days in the earlier decade to 32 in the more recent.
The new research was led by Robert E. Kopp, an earth scientist at Rutgers University who has won respect from his colleagues by bringing elaborate statistical techniques to bear on longstanding problems, like understanding the history of the global sea level.

Based on extensive geological evidence, scientists already knew that the sea level rose drastically at the end of the last ice age, by almost 400 feet, causing shorelines to retreat up to a hundred miles in places. They also knew that the sea level had basically stabilized, like the rest of the climate, over the past several thousand years, the period when human civilization arose.

But there were small variations of climate and sea level over that period, and the new paper is the most exhaustive attempt yet to clarify them.

The paper shows the ocean to be extremely sensitive to small fluctuations in the Earth’s temperature. The researchers found that when the average global temperature fell by a third of a degree Fahrenheit in the Middle Ages, for instance, the surface of the ocean dropped by about three inches in 400 years. When the climate warmed slightly, that trend reversed.

“Physics tells us that sea-level change and temperature change should go hand-in-hand,” Dr. Kopp said. “This new geological record confirms it.”

In the 19th century, as the Industrial Revolution took hold, the ocean began to rise briskly, climbing about eight inches since 1880. That sounds small, but it has caused extensive erosion worldwide, costing billions.

Due largely to human emissions, global temperatures have jumped about 1.8 degrees Fahrenheit since the 19th century. The sea is rising at what appears to be an accelerating pace, lately reaching a rate of about a foot per century.

One of the authors of the new paper, Dr. Rahmstorf, had previously published estimates suggesting the sea could rise as much as five or six feet by 2100. But with the improved calculations from the new paper, his latest upper estimate is three to four feet.

That means Dr. Rahmstorf’s forecast is now more consistent with calculations issued in 2013 by the Intergovernmental Panel on Climate Change, a United Nations body that periodically reviews and summarizes climate research. That body found that continued high emissions might produce a rise in the sea of 1.7 to 3.2 feet over the 21st century.

In an interview, Dr. Rahmstorf said the rise would eventually reach five feet and far more — the only question was how long it would take. Scientists say the recent climate agreement negotiated in Paris is not remotely ambitious enough to forestall a significant melting of Greenland and Antarctica, though if fully implemented, it may slow the pace somewhat.

“Ice simply melts faster when the temperatures get higher,” Dr. Rahmstorf said. “That’s just basic physics.”

As is known by our people, the World Health Organization has warned of the rapid spreading of the Zika virus, affecting 32 countries on our continent, and has declared an international health emergency.
Although scientific evidence has not demonstrated that a Zika virus infection during pregnancy causes microcephaly, in all cases reported in affected countries, a strong link with an increase in this birth defect, yes, has been found.
The disease is spread by mosquitoes from the genus Aedes. There is currently no vaccine or treatment for the illness, therefore the best way to prevent it is to eliminate or reduce the vector’s presence, and protect ourselves from these insect bites.
To date, no cases have been detected in our country, although all cases of non-specific fever symptoms are being monitored and studied to identify the presence of the virus early on, and international sanitary controls are being intensified. If anyone with the virus is found, all conditions are in place to provide qualified medical assistance.
Cuba has undertaken intense efforts to reduce the vector’s presence, with the objective of maintaining its presence at levels which do not represent a danger.
Nevertheless, factors related to inadequate technical quality of anti-vectoral work, poor environmental sanitation, and adverse weather conditions have contributed to the risk of diseases spreading.
Given this situation, the Party and government leadership have adopted a plan of action, under the direction of the Ministry of Public Health, to confront the Zika virus, and consequently dengue and Chikungunya, as well.
Once again, the true leadership of the struggle against the threat of an epidemic is the responsibility of our entire people, since their conscious participation is indispensable if this important and necessary task is to be successful.
Urgently needed are intensive sanitation programs in workplaces, residential areas, and inside homes themselves, with the active collaboration of community bodies and grassroots organizations.
It is imperative that every Cuban man and woman assume this battle as a personal issue, a worrisome problem, given above all the responsibility it implies for their families.
As part of the approved action plan, the Revolutionary Armed Forces is assigning more than 9,000 troops, including permanent and reserve officers, as well as technical equipment to reinforce anti-vectoral and sanitation efforts, with the additional support of 200 Revolutionary National Police officers.
At this time, it is imperative that all citizens and entities strictly comply with sanitation norms and measures adopted to guarantee anti-vectoral efforts, to avoid contributing to the spreading of epidemics, or other behaviors which promote or constitute a threat to public health.
Following this intensive stage, a plan must be implemented which is sustainable over time, that does not become just another campaign, to guarantee that levels of infestation are minimal.
As has been the case on other occasions, our people will be able to show their capacity for organization, to maintain the levels of health reached by the Revolution, and thus avoid suffering for our families. More than ever before in similar efforts, it is imperative to be more disciplined and exacting.

Committees for the Defense of the Revolution ready to Confront Public Health Emergency

HAVANA.— Committees for the Defense of the Revolution (CDR) across the country will undertake sanitation hearings and preventative measures February 26 through March 1, in response to President Raúl Castro’s call to energetically confront the current epidemiological situation.
To be distributed today in all provinces and municipalities is a document outlining measures to be adopted by Cuban families to combat Aedes mosquitoes and avoid epidemics, explained Teresa Ramírez Paz, who attends public health issues at the CDR national office.
Speaking to ACN, she reported that community leaders of CDR, the Federation of Cuban Women, and People’s Power bodies, as well as family doctors and nurses will receive training to prepare for the hearings.
Public health experts will participate in these meetings in locations facing serious mosquito infestations, she said.
Among steps to be taken by CDR are accompanying sanitation inspectors, supporting fumigation teams, and eliminating all possible mosquito breeding sites.
More than 30 countries in the Americas have reported cases of Dengue, Chikungunya and Zika; while Angola is currently experiencing a serious malaria outbreak.

2218. Fernando Cardinal Dies at 82

MANAGUA, Nicaragua — Fernando Cardenal, one of the priests who famously rebuffed a papal order to quit Nicaragua's leftist Sandinista government, died in Managua on Saturday. He was 82 and long back in the good graces of the church.

The Jesuit was a practitioner of liberation theology and he who joined the Sandinista rebels after they toppled the dictatorship of Anastasio Somoza in 1979, first leading a widely praised literacy campaign and then serving as education minister.

The participation of priests such as Cardenal, his brother Ernesto and Miguel d'Escoto in the leftist government irked Pope John Paul II, who publicly wagged his finger at Ernesto Cardenal during a papal visit to the country in 1983. A year later, they were among four suspended from the priesthood for refusing to leave their political posts. Ernesto Cardenal, perhaps Nicaragua's best-known poet, was culture minister and d'Escoto was foreign minister.

In an open "Letter to my Friends" published in late 1984, Fernando Cardenal defended his decision in theological terms, saying, "I believe before God that I would be committing a grave sin" by abandoning a post committed to the poor.

"I cannot conceive of a God that would ask me to abandon my commitment to the people," he said.

Fernando Cardenal left his Cabinet post in 1990 and, like his brother, eventually broke with the main Sandinista Front led by Daniel Ortega.

In a step even more unusual than his suspension, he was reinstated to the Jesuit order in 1997 and until his death was heading the Nicaragua office of Fe and Alegria, a Jesuit program that brings schools to poor parts of Nicaragua and other countries.
The agency confirmed the death in a statement, praising his "tireless struggle to defend the rights of the most vulnerable.”

Local news media said the death resulted from complications following a surgery two weeks ago.
